Looking for the chord matrix from Paul Reisler's Chord Substitution class

We used this resource during his class and I'd really love to have it.  Paul gave us a link to the matrix (or he might have called it a chord web or substitution chart...), but I must have written it down incorrectly.  Here’s what I wrote (and doesn’t work):

Does anyone know the correct link or have another way to get this chart?

Hi Diana,
  I've attached a couple jpg's of what I copied in my notes during the class.  I have a powerpoint also if you're interested, I can email or post to sharepoint so you can download.  Don't know if its all correct, maybe Paul can take a look.
